New Patients: Scheduling Process

See the scheduling calendar for phone consults, virtual visits, and in-person visits at our office space in Hopkins, MN. Our office is located in Reverie Mental Health PLLC at 32 10th Ave S, Suite 206, Hopkins, MN 55343.

   Schedule a free phone consult
   15 minutes

   This is required. We will not be able to confirm an intake without first having completed a phone consult with you.

2. 02
   Schedule an intake
   75 minutes

   It is up to you whether you would like to schedule an in-person or telehealth/virtual visit. However, some medication requires that you be evaluated in-person at least once in 2024. If we think one of these medications might be part of your treatment plan, we will discuss scheduling an in-person visit when it is convenient for you.

3. 03
   Follow up visits
   30 or 45 minutes

   At the end of your intake or follow up visit, MacKenzie will recommend when your next follow up should be.

Current Patients: Portal

We use ChARM, a web-based electronic health record (EHR/EMR), as the primary means of contact between patient and provider.

You can use it to:
• securely ask questions or communicate about your treatment plan.
• see your prescribed medicines, lab results, chart notes, invoices, etc.
• schedule appointments and request refills.

You can create an account on ChARM after scheduling your phone consultation. If you need an invitation to make a portal account and you have not received one within 2 days of your appointment, please contact us to request your invitation.
